A rare case of metastatic prolactinoma presenting with cervical myelopathy

Bashiti Hazem , Broughton Chloe , Nelson Richard

Case History: A 52 year old man presented in 2009 with a gradual loss of peripheral vision associated with headaches. Examination confirmed bi-temporal hemianopia. Further investigation revealed elevated prolactin levels and pituitary MRI showed evidence of large pituitary tumour. A diagnosis of macroprolactinoma was made and cabergoline was commenced. Given resistance to medical treatment, the patient required two surgical resections (2011 and 2013) and radical radiotherapy (...

An unusual presentation of thyrotroph adenoma

Raghavan Rajeev , Plaha Puneet , Nelson Richard , Lightman Stafford , Levy Andrew

Background and case: A 38-year-old man presented acutely with left-sided retro-orbital pain, a heavy/numb sensation in his left arm and blurred vision and diplopia on left lateral gaze. Baseline biochemistry, liver function and haematology were normal. His TSH was 3.9 (0.3–4 mU/l) and CRP 92 mg/l (<10). Plain CT head was unremarkable as was the MRI as reported, and atypical migraine was suspected. Vertical diplopia on left lateral gaze and left upper arm paraesthesia ...

Pituitary gonadotrophinoma and ovarian-hyperfunctioning: link or coincidence?

A 40-year-old woman, who had developed oligomenorrhoea after discontinuing the contraceptive pill at age 30, presented with a 6-month history of intermittent vomiting associated with abdominal discomfort and distension. Ultrasound, followed by laparotomy, revealed multiple bilateral ovarian luteal cysts consistent with hyperoestrogenism and ovarian-hyperfunctioning.Only 9 months post-operatively her bilateral theca ovarian cysts had recurred. Over 4 mont...

Pituitary adenomas comprise a heterogeneous group of tumours causing serious comorbidities, which would benefit from identification of novel, common molecular/cellular biomarkers and therapeutic targets. The ghrelin system comprises a complex molecular family with multiple functions, and some of its components have been linked to development of various endocrine-related cancers. In this work we aim at better delineating the patho-physiological significance of the ghrelin regul...
